Practical Examples of How We Help

A few typical situations we are asked to help with. These are general examples only - every plan, and every person, is different.

  • A first plan, and no idea where to start

A participant receives their first NDIS plan and is unsure what the funding categories mean or which providers to approach.

How we may help: We walk through the plan together in plain language, shortlist providers that suit the participant's goals and preferences, and help set up the first service agreements.

  • Several providers, all working separately

A participant has therapy, support work and health appointments running at once, with no one holding the overall picture.

How we may help: We bring the providers into a shared approach with clear responsibilities, so appointments and reports line up instead of overlapping or falling through the gaps.

  • A housing situation that is becoming unstable

A participant's living arrangement is at risk, and the supports around them are not set up to respond quickly.

How we may help: We coordinate with housing, health and disability services, map the risks ahead of time, and help the participant understand the options available to them.

  • Mental health that changes week to week

A participant living with psychosocial disability finds that supports which work one month do not suit the next.

How we may help: Through Psychosocial Recovery Coaching we pace the work around the participant, adjusting supports as things change and keeping the focus on their own recovery goals.

  • A plan reassessment coming up

A participant is approaching a plan reassessment and is worried about explaining what has and has not worked.

How we may help: We help gather evidence, record progress against goals, and prepare the participant to describe their support needs clearly in their own words.

These examples are illustrative and de-identified. They do not describe individual participants and are not a guarantee of any particular outcome.

  4. 4

We review goals, needs and available funding

We look at the participant's goals, their support needs and the funding available in their NDIS plan.

  5. 5

We prepare the service agreement and commence services

If the participant chooses Nexa, we put a service agreement in place and get started.

Funding and Fees

Our services are delivered using the funding already in your NDIS plan. There is no separate fee to you on top of your plan funding.

  • Specialist Support Coordination

    Capacity Building - Support Coordination

A separately funded, higher-intensity level of coordination for participants whose situation involves significant complexity or risk.

  • Psychosocial Recovery Coaching
