Specialist Support Coordination
Specialist Support Coordination is for participants whose circumstances involve a high level of complexity - where multiple services, significant risks, or barriers in the support environment are stopping a plan from working. It is delivered by an appropriately qualified practitioner and is usually time-limited: the goal is to reduce the complexity so that more standard supports can take over.
What Specialist Support Coordination Involves
Addressing Barriers in the Support Environment
Identifying the specific obstacles - housing, health, justice, family circumstances or a breakdown between providers - that are preventing a plan from working, and putting a strategy in place to resolve them.
Designing and Implementing a Coordinated Plan
Bringing multiple providers and service systems into one agreed approach, with clear responsibilities, so that supports stop working at cross purposes and the participant is not left holding it together.
Managing Risk and Responding to Crisis
Planning ahead for foreseeable risks, and coordinating a fast, organised response when a participant's situation escalates, so that decisions are made calmly rather than under pressure.
Building Capacity and Resilience
Working steadily to reduce the level of specialist involvement needed, so that supports become more stable and can be maintained through standard Support Coordination or by the participant and their network.
